Regardless of your reason, these plates are a unique way to show your support all year long for the quality patient care at Lake Taylor Transitional Care Hospital. Proceeds from the sale of the special plates will benefit the Foundation, a certified 501 (c) (3) charity, and will help raise needed funds for the Foundation’s goal to improve the quality of life in our community and provide the best healthcare possible.

The Virginia Department of Motor Vehicles will develop official state license plates using the following Mermaid design, and the motto, “Life. Celebrated Daily.” The plates are available to any resident of Virginia; you needn’t be a Norfolk resident to purchase one. (The proposed design is pending DMV approval and subject to minor changes.)

Print the official DMV application form (VSA 10) from the following link: http://www.dmv.virginia.gov/webdoc/pdf/vsa10.pdf

1. In the “Plate Information” section, check the “Other” box, and write: “Mermaid”
2. If you want new or retained personalized characters (a separate additional charge) complete the personalized plate section as usual
3. Provide a check of $25 made payable to the Lake Taylor Transitional Care Hospital Foundation and put “License plates” in the memo line
4. DO NOT send to the state DMV in Richmond
5. Drop off or send payment and completed form, directly to the following:

6. Credit Cards are also ACCEPTED. If paying by credit card, you can fax your completed and signed application to 461-4282. Please make sure your credit card number is legible!

When the plates become available, the DMV will send plates via the US mail. Each year, renewing your plates will cost an additional $25, $15 of which will be shared with the LTTCH Foundation.
